help with a workout log

What all do i need to contain in a workout log in order to monitor results, overtraining, overtiredness, and the overall feel good, "im in the best shape of my life" feeling after every workout....

Sets and reps, weight...anything particular that im missing

I just make a chart using the chart function in MS word. I put three days on each page. I plan each set, rep, pound ahead of time in pencil so I can change things if I need to. Each week has one sheet of paper. There is room at the bottom to write extra notes like "trouble with this one", "Repeat rows next time instead of increasing weight", "overhead press was too easy, move hands closer next time". Just stuff like that. Between 8-12 weeks of papers on my clipboard and my daily online workout journal I'm able to keep myself on track.

Maybe you can do something like that? Or do an excel spreadsheet?
