to go to failure means you absolutely CANNOT do one more rep. usually you know this because you had to crawl out from under the weight or the spotter had to grab it and rack it.

I like to refer to them simply as "sets".

There is also such thing as positive failure. Which basically is the most you can do, with good form. For example, you may be able to do a weight 12 times, but can only do it with good wholesome form maybe 8. So positive failure would be to get the weight up with good form, 8 times.
