School Health Survey


I am a student conducting research on sports and health beverages. Any responses will be greatly appreciated. Please download and return the following completed questionnaire.

I'll be using similar quantitative information gathering tools for my dissertation on creatine usage in resistance athletes for various online forums and it's a pity feedback was poor on DF.

I feel that my body obtains all of the nutrients it requires from my quality wholesome foods.
